研究主題

本實驗室之研究主題近年來著重於細菌分子鑑定與抗藥機轉。
以下列出部分過去的研究及成果。

一、尋找新的基因標的，發展快速測定方法
   近年來細菌的分子鑑定已日趨普遍，然而最常用的16S rRNA基因對演化太相近的菌種常常無法區分，因此尋找其他基因作為菌種鑑定的標的也是很重要的。自2001起我們即致力於groESL基因的研究，發現在許多菌種中，groESL基因可以補助16S rRNA 基因之不足，甚或作為唯一的基因標的。此類研究成果包括腸球菌(Teng et al. 2001 及Tsai et al. 2005)；草綠色鏈球菌 (viridans group streptococci) (Journal of Clinical Microbiology; Teng et al. 2002)；Streptococcus mutans (Journal of Medical Microbiology; Hung et al. 2005)與Streptococcus bovis (2008)等；結果均顯示菌種間之groES、groEL、spacer序列均有明顯差異。同時我們亦設計Streptococcus-specific PCR，直接應用於臨床檢體(腦膿瘍)之偵測 (Tsai et al., 2004; Neurosurgery)。

二、 抗藥機轉的研究
    細菌抗藥機轉非常複雜，且常因不同細菌、不同地區有不同的抗藥型式或機轉。自1998年至今，本人一直長期分析及探討鏈球菌之抗藥型式，近年來發現Streptococcus bovis group對macrolide的抗藥型式與其他鏈球菌有明顯不同，且發現是因帶有erm(T)基因所致，此成果發表於Antimicrobial Agents and Chemotherapy (Teng et al. 2001) ，此報告為文獻上第一篇在鏈球菌發現帶有erm(T)者。申請人進一步分析Streptococcus bovis erm(T) 基因，發現erm(T)抗藥基因二端為IS1216V序列，並以I-CeuI證實位於染色體核酸上(Antimicrobial Agents and Chemotherapy, 2005)。此外由於近年來多重抗藥Pseudomonas aeruginosa菌株明顯增加。為了解對carbapenem抗藥基因散播的機制，我們針對帶有blaVIM-3的抗藥菌株進行其flanking之基因結構，發現blaVIM-3是位於Integron In450上，而In450是位於轉位子Tn6001中，大小約14 kb。此研究更利用I-CeuI切割及脈衝電泳，證實此抗藥結構位於細菌染色體上。Tn6001與Tn1403或Tn1404*最接近，但Tn1403或Tn1404*並未被報導過有帶blaVIM-3的抗藥基因。因此轉位子應是造成Pseudomonas aeruginosa 對carbapenem抗藥增加的原因。其他抗藥研究還包括methicillin-resistant Staphylococcus  aureus 的基因型分析(Clinical Microbiology and Infection 2007; 13:717-724)，發現醫院中的社區型感染菌株有增加趨勢。

三、光動力殺菌
    光動力為光感物質經過光的誘發後，產生有毒性的單態氧，造成細胞死亡。近年來除了用於腫瘤的治療外，亦開始探討在微生物感染的治療。我們針對一些抗藥性強的細菌，測試光動力殺菌效果，發現某些細菌雖具有較強的efflux pump，卻不影響光動力殺菌的效果。
