研究主題

  1. 以基因分析第二型人類白血球抗原之變異。利用聚合酶鏈反應(PCR)配合序列特異性寡核苷探針點雜交技術(SSOPH)解析具高度變異性的第二型人類白血球抗原基因,用於調查台灣地區閩南、客家、外省及泰雅、賽夏、鄒、阿美等原住民族群之HLA變異以探討彼此間的血緣關係;並以此系統成功參與或指導多個HLA與多種疾病(第一型胰島素依賴性糖尿病、家塵螨過敏反應、乾癬、慢性C型肝炎發展自體免疫、HBV帶原者發展慢性肝炎、與持續HPV感染關聯之子宮頸癌等)相關之研究。

  2. 自體免疫疾病(SLE,類風濕性關節炎)的分子標記研究: 自體免疫疾病為免疫系統調節異常引起的自我破壞攻擊所致全身性系統疾病。 雖然罹病者的免疫系統異常已有許多研究並發展成常規的檢驗,自體免疫疾病的疾病誘發原因至今仍未完全明瞭,且疾病的表現具有異質性,不僅癥狀變化多樣,對於治療的反應及疾病的控制的情形亦有所不同。已知環境因子(如:感染)與生理變化因子(如:荷爾蒙)與疾病發生或疾病的進展有關,同時自體免疫疾病有家族聚集傾向,顯示某些遺傳因子的差異可能與罹病或疾病的發展有關。找尋與罹患自體免疫疾病或與罹病者特定疾病臨床表現或病程發展相關的分子標記,將有助於探究自體免疫疾病的成因,並有助於發展有效的疾病控制治療方法。我們由探尋凋亡細胞清除相關因子(如: MFG-E8, Gas 6, Stabilin2, HMGB1等),檢測這些因子之表現變化、基因變異與自體免疫疾病(系統紅斑性狼瘡、類風濕性關節炎等)發生的關聯性,並和特定的病程變化做連結,找尋新的自體免疫疾病基因標記、探討疾病發生機轉。 我們發現特定的MFG-E8, Stabilin2,或HMGB1基因變異型與罹患紅斑性狼瘡有顯著正相關,進一步研究這些基因變異對相對的分子表現及其結構/對凋亡細胞清除功能的影響,可以了解凋亡細胞清除異常是否在人類的自體免疫疾病發生中扮有一定的角色,未來亦可能針對屬於凋亡細胞清除異常而致病的SLE病人設計治療以減緩疾病的惡化。與正常血清相較,SLE病患血清中有較高的Gas6,且SLE病患中,有發展為腎炎(lupus nephritis)的病人血清中Gas6更高,Gas6可作為SLE疾病活性(disease activity)的標誌分子,Gas6的增加是否參與SLE的疾病發展有待更進一步研究。

  3. 兒童急性白血病相關研究:

    • 台灣地區兒童急性白血病化學治療之藥物基因學研究: 兒童急性淋巴性白血病是兒童常見的癌症。現今的治療採依病人的發病年齡,血液中血癌細胞數及癌細胞所帶的基因變化種類所做的復發風險分群(標準,高,極高風險)給予不同藥物組合/劑量的標準治療方式。目前對於標準風險與高風險群病人的治療結果有85%病患可達5年以上存活或痊癒,極高風險群病人即使施以最高劑量的複合式化學治療,仍有近七成病人短期內在發並對後續治療失去反應並惡化死亡。臨床上相同風險分群病人接受相同治療療程,但並非所有病人皆有相同的治療結果,由於多數病患的疾病復發皆發生於鞏固治療或維持治療時期, 推測或許某些與這兩治療期用藥物methotrexate(MTX,甲基葉酸)與Thiopurine的代謝或排除能力相關的分子差異是造成治療結果不同的原因。我們收集105名2002年以前接受治療的小兒ALL樣本,針對12個與MTX或Thiopurine代謝/排除相關分子的基因上已有報導與其酵素表現/功能有關的16個變異基因位點作分析,並與治療結果(有無復發死亡)做比對分析。發現其中在標準風險群36例中,MDR1 2677/3435位點帶有GG /CC 基因型的病人在接受該組治療法後有50%在三年內發生疾病復發,是為血癌復發獨立風險因子(Hazard ratio: 6.8),此二位點雖不在MDR1基因的轉譯序列上,但有研究指出其與產物ABCB1藥物幫浦分子的的高表達與藥物排除有關。此一發現更可由分析台灣地區2002年後小兒ALL治療藥物劑量調升後,30例標準風險群病人在3年內沒有復發病例得到佐證,顯示提高化療劑量後可克服MDR1基因型變異造成的藥物排除效力差異與治療失敗。對於極高復發風險群病人已施用最高劑量的化療,仍有半數病人在三年內復發,藥物基因變異型與疾病復發與否沒有明顯關係,可見現行的化療對於極高復發風險群的ALL治療仍有死角,無法完全符合治療的需求,有待開發其他治療的藥物。

    • 天然小分子藥物對於抑殺白血病細胞之效能與其分子機轉: 帶有t(4;11)或t(9;22)染色體轉位變化的急性淋巴性白血病具有極高復發風險(very high risk, VHR),臨床上施用最高劑量化療仍有高比例病人無法達成緩解或短期內即發生血癌復發而對治療發生抗性,病程加劇而死亡。t(9;22) Bcr/Abl-ALL在成人(尤其是老人)的ALL中比例大增,雖然併用tyrosine kinase inhibitor可以延長緩解時間數月,多數病人仍不免疾病復發。因此找尋有別於現行化學治療的抑制血癌藥物並研究其中抑殺血癌細胞的分子機轉有助於發展新一代的對抗VHR-ALL治療或設計現行治療的增敏劑。我們發現萃取自生漆的天然小分子化合物 HQ17(3) [10(Z),13’(E),15’(E)- heptadecatrienyl-hydroquinone]在低濃度下對於多株人類急性白血病細胞株具有快速的抑殺作用但對正常血液中的白血球不具毒性。針對帶t(4;11)之 RS4;11與帶t(9;22)之SUP-B15 ALL細胞,我們發現HQ17(3)皆可造成細胞膜磷脂外翻、細胞膜破壞/通透增加、粒線體膜電位喪失,及染色體DNA斷裂等細胞傷害及死亡。使用抗氧化劑可大幅降低HQ17(3)在ALL細胞造成的細胞傷害及死亡,但是使用caspase抑制劑並未減少之。因此HQ17(3)在ALL細胞中引起的快速抑殺機制與細胞內氧化壓力增加有關,但卻非透過已被廣泛研究、使用依賴caspase活化的細胞凋亡途徑。進一步解析參與HQ17(3)抑殺VHR-ALL細胞的分子機轉將助於找出重要分子,有潛力發展出新一代的對抗VHR-ALL治療或設計現行治療的增敏劑。
